LICENCE UNDER SECTION 24 (1)

Whereas it has been proved to my satisfaction:

(a) that an association to be entitled ............................................. (hereinafter referred to as the said .............................................) is a to be formed as a limited company for the promotion of a charitable and useful objects set out in the Memorandum of Association of the said .............................................

(b) that the said ............................................. intends to apply the whole of its profits and other-income in the promotion of the said charitable and useful objects,

(c) that the said ............................................. is prohibited by its constitution from the payment of any dividends to its members.

Now herein I, ............................................. Minister of Trade and Industry do hereby direct by virtue of the powers conferred upon me by section 24 (1) of the Companies Act, 1965, that the said ............................................. be registered as a company with limited liability without the addition of the word "Berhad" to its name.
